Overview $1,500 Sign‑On Bonus. Flexibility, competitive pay, and a supportive culture. Schedule visits Sunday‑Saturday 8 am‑7 pm based on your availability.
Responsibilities
Conduct adult/geriatric assessments (medical history, diagnosis, treatment, health education, referrals, case management, follow‑up, clear documentation).
Administer and order point‑of‑care and lab‑based testing as needed.
Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
Collaborate with primary care physician on patient education and follow‑up.
Provide services at the member’s residence (home or skilled nursing facility).

Competitive pay – $90–$115 per assessment, potential $1,800–$11,500 per month.

Qualifications
Master’s degree or equivalent experience and valid NP licensure.
Current NP licensure with prescription authority (or ability to obtain it).
Board certification AANP, AACN, or ANCC in adult/gerontology/acute care/family/emergency specialty.
Current BLS, ACLS or CPR certification.
1 year NP experience preferred; new graduates encouraged.
Valid state driver’s license and proof of adequate auto insurance.
Strong computer skills and familiarity with electronic health records.
Excellent verbal and written communication skills.
