MHMR of Tarrant County is looking for a skilled Service Coordinator. Each person receiving services in the HCS, TxHmL, and GR CFC will have a single identified staff member, called a Service Coordinator (SC). The SC will assist the person in identifying and articulating his/her priority outcomes, personal goals, and hopes for the future, as well as his/her requests for services and supports. The SC will elicit the person's feedback, coordinate, and monitor the services to achieve desired outcomes as identified in the Person Directed Plan (PDP). The goal of a Service Coordinator is to provide the people we serve with seamless, consumer‑driven services and support. The SC must also be knowledgeable about the array of services and supports available in the community.
Responsibilities
Identify and articulate client priorities, goals, and hopes for the future.
Elicit client feedback and coordinate services.
Monitor services to ensure achievement of desired outcomes per the Person Directed Plan (PDP).
Provide seamless, consumer‑driven services and support.
Maintain knowledge of community services and supports.
Minimum Requirements
Minimum Education:
Bachelor’s Degree in Health or Human Services.
Acceptable Degrees Include:
Applied Behavior Analysis, Behavioral Services, Communication Disorders, Criminal Justice, Counseling, Education, Family Studies, Gerontology, Educational Psychology, Health Services, Human Services, Human Growth and Development, Medicine, Nursing, Physician Assistant, Psychology, Rehabilitation, Social Services, Social Work, Sociology, and Special Education.
Required License:
Valid Texas driver’s license with an acceptable driving record.
Must provide license (if applicable) and official documentation of all educational attainment (Transcript/Degree/Certificate).
Preferences:
Bilingual a plus but not required.
