Responsibilities

Conduct adult / geriatric assessments including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow‑up, and clear documentation.

Administer and order point‑of‑care and lab‑based testing as needed.

Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.

Collaborate with Primary Care Physicians on patient education and follow‑up.

Provide services at the member’s residence, whether in home or skilled nursing facilities.

Competitive Pay – Earn between $1,800–$11,500 per month based on the number of assessments (average 20–100 visits).

Qualifications

Master’s degree or equivalent experience and NP licensure.

Current NP licensure in practice state with prescription authority or ability to obtain it.

Board certification (AANP, AACN, or ANCC) in a relevant specialty.

Current BLS, ACLS, or CPR certification.

1 year NP experience preferred; new graduates encouraged.

Valid state driver’s license and proof of insurance.

Strong computer skills and familiarity with employee health/medical record software.

Excellent verbal and written communication skills.
